The Fond du Lac Family YMCA announces a new online application process for their financial assistance program, Membership for All.
The Y opens their doors to everyone in the community and believes that no one should be denied access based on their ability to pay. Through the Membership for All program, the Fond du Lac Family YMCA provides assistance to youth, adults and families based on individual needs and circumstances. Any individual or family whose total combined gross household income is less than $80,000 per year is encouraged to apply. Monthly membership rates are based on total household income.
How to Apply Online
To apply online, complete the online Membership for All application found at fdlymca.org and upload all income verification documents, as applicable. After your application is reviewed you will receive an email within 1-3 business days, indicating whether your application was approved or denied. Membership for All approvals require membership enrollment within 30 days of approval, or re-application is required.
Submit these required documents with application:
- Most recent federal income tax return (1040 or equivalent, not W-2 forms) is required for all adults (not including dependents) in the household.
- Last two pay stubs for all adults (not including dependents) in the household.
Submit these documents with application as applicable:
- If receiving Supplemental Security Income (SSI) or disability benefits, then submit your most recent Social Security Statement.
- FoodShare benefits statement.
- Child support and/or alimony monthly documentation.
- Unemployment monthly compensation statement.
- Pension monthly benefit statement.
- Documentation with names of eligible person(s) for subsidized housing statement.
Verification of residence may be required for all those listed on the membership.
Membership for All reduces membership fees; it does not eliminate them. Membership staff will determine your rate and set up your payment plan.
Printed applications are found online at fdlymca.org or can be picked up at the YMCA at 90 W. Second St., Fond du Lac. Return completed application and required copies of proof of income to the Fond du Lac Family YMCA.
A payment or billing method must be provided to establish membership. Membership payments may be made upfront in full for 12 months or may be drafted monthly out of a checking or savings account. A blank check or bank statement indicating account numbers is required, or monthly drafts may be established using a debit or credit card.
